A later observation conflicts with an earlier state without erasing it.

A required fact, source, or link is absent.
Source dependence
Repeated claims trace back to one underlying source rather than independent evidence.
Unresolved contradiction
The system preserves the conflict as candidate evidence until governed review resolves—or leaves—it open.
